We are out of network for the following insurances. We will file for you as out of network (going towards your out of network deductible) but require payment at the time of service:
Payment for assessment and treatment sessions are due at the time of service. Payment is accepted in the form of check (pre-paid to office) or credit card on file. For a complete outline or private payment fees, please contact us at firstname.lastname@example.org.
Credit Card Policy:
All clients will complete a credit card permission form. A credit card will be kept on file for all clients for charging of individual sessions, deductible payments, co-pays and co-insurance. A credit card will also be utilized in case of no shows and to be charged weekly (with parents’ permission). If credit card is declined, sessions will be held until sessions are paid in full.
Cancellation/No Show Policy:
All appointments that are missed without notification 24 hours before will be charged the session fee for the missed appointment. Patient will not be seen if late cancellation fee has not been paid.
